Create a new user profile

Please note: the "Login Name" for all client-wiki accounts must conform to the following format:

<functional_title>.<county>.<orgcode>

Valid orgcodes are:

c: Court
s: State's Attorney
p: Probation

Examples:
Login name/ID: deputy_clerk_traffic.logan.c or administrative_liaison.logan.c

Additionally, your "Name" should be indicative of your functional title as well. Examples:
Name: Logan County Circuit Clerk Deputy Clerk, Traffic or Logan County Circuit Clerk Administrative Liaison

Your account will not be activated until the administrator approves your request. Adhering to the above guidelines will speed up this process.

Password *
Password verify *
Name *	Use your name in Access Control Lists or wiki Groups.
E-mail address	(optional). If you lose your password, you can ask to have a new, random password sent to this address.
